Getting started as a clinical academic Health Care Professional

Clinical academia allows registered and regulated Health Care Professionals (HCPs) to combine clinical work with research, teaching and leadership. There isn’t a single route, careers can be flexible and tailored to your interests.

Typical career stages include:

  1. Pre-doctoral: Build research skills, shape a research question, secure mentorship and prepare a competitive doctoral application.
  2. Doctoral (PhD): Conduct original research linked to clinical practice while maintaining some clinical hours.
  3. Post-doctoral: Consolidate independence, lead studies, win grants, supervise and translate findings into practice and policy.

Opportunities to explore early research

While early exposure to research is not mandatory, it can help clarify if a clinical academic career is right for you. It can also strengthen future applications and allows you to build practical skills alongside patient care.

NIHR INSIGHT programme (England only)

Many NHS trusts, Research Delivery Networks (RRDNs) and universities offer short-term internships to develop research skills and experience real-world clinical studies. Outside England, check your local Health Board or university research office.

Research fellowships

These provide salaried or partially salaried time to build research and leadership experience while undertaking a defined project. Baseline requirements usually include registration with your professional body (NMC or HCPC), employer support for protected time and academic supervision with a host university.

Clinical audits, quality improvement and teaching

Even without formal fellowships, engaging in audits, teaching and service improvement projects builds essential research skills while remaining clinically active.

Pre-doctoral opportunities by nation

Predoctoral opportunities vary across the UK, with each nation providing schemes that reflect local priorities and funding structures. They are ideal for health professionals who want to:

  • build research skills while remaining clinically active
  • test out research ideas in real-world settings
  • network with academic mentors and research teams
  • prepare competitive applications for doctoral-level funding

The National Institue of Health Research (NIHR) offers structured pre-doctoral routes for aspiring clinical academics. These include:

  • NIHR Predoctoral Award: Tailored training to develop your research skills up to master’s level, preparing you for doctoral study.
  • NIHR In-Practice Fellowship (IFP): Pre-doctoral training for HCPs working in primary care, with a focus on research and education.

The primary route to qualification in Scotland is via the Chief Scientist Office (often in conjunction with NHS Education for Scotland).

  • CSO NHS Researcher Development Fellowship: Designed for NHS employees in nursing and allied health professions. The fellowship can provide a comprehensive introduction to research or support those with experience to develop advanced skills.

RCBC Wales “First into Research” Fellowship: A 12-month programme (1 day/week) for clinicians with little or no research experience, helping build confidence and skills for future doctoral applications.

  • NIHR Pre-Doctoral Fellowship: Supports Master’s-level study, research training and project development (12-month FTE). Applicants must secure prior approval from the HSC R&D Division.
  • Workshops and masterclasses: NIHR runs targeted sessions for nurses, midwives, and AHPs to develop research and clinical skills.

  • Many specialty societies, such as the RCN Research Society, offer small grants, mentoring and networking opportunities.
